Key Responsibilities

Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
Utilize standardized assessments and evidence-based practice to guide clinical interventions including manual techniques, therapeutic exercises, ambulation, modalities, assistive devices, and wound care
Monitor and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes in the electronic documentation system per regulatory requirements
Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
Participate in required meetings such as patient care conferences, utilization review meetings, and rehabilitation meetings, as well as required trainings and in-services
Supervise and support physical therapist assistants, rehabilitation aides, and/or students as applicable

Required Qualifications

Active Missouri State Physical Therapist license
Minimum of 1 year of physical therapy experience, or new graduate with completed Level II clinical experience
Long Term Care (LTC) or rehabilitation setting experience
Adherence to APTA and Missouri state practice acts, code of conduct, and professional ethics

Preferred Experience / Skills

Experience with electronic documentation systems in a clinical setting
Familiarity with DME procurement and wound care protocols within a long term care environment
